Platform and Provider Roles
OptimizeMD is operated by C2C Ventures LLC and facilitates access to independent healthcare providers. OptimizeMD is a telehealth platform and administrative services provider, not a medical practice.
Medical services are provided by independent licensed healthcare professionals associated with LocumTele and Providers On Demand. These healthcare professionals exercise independent medical judgment and are responsible for clinical evaluation, diagnosis, treatment recommendations, and prescribing decisions. Availability and licensure vary by jurisdiction.
What Telehealth Means
Telehealth may involve electronic communications, medical questionnaires and intake forms, secure messaging, audio or video visits where applicable, transmission of medical and laboratory information, and remote evaluation. An appropriately licensed physician or other licensed healthcare professional acting within applicable scope-of-practice law may provide care.
Potential Benefits
Telehealth may improve convenience and access, reduce travel, and allow remote communication with a healthcare professional. These are potential benefits only; telehealth does not guarantee a better or particular outcome.
Limitations and Risks
- A remote provider cannot perform every physical examination available in person.
- Evaluation depends on complete and accurate information from you.
- Technology failures, interruptions, delays, or privacy and security incidents may occur.
- An in-person examination, testing, referral, or emergency care may be required.
- Treatment may be declined, and a prescription is not guaranteed.
- Provider and treatment availability are limited by licensure, jurisdiction, and applicable law.
No Emergency Services
Telehealth services available through OptimizeMD are not for emergencies. If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.
Your Responsibilities
You agree to provide truthful, accurate, and complete information; disclose relevant symptoms, conditions, medications, and health history; follow provider instructions; ask questions when information is unclear; and seek in-person or emergency care when directed or appropriate.
Independent Clinical Judgment
Your healthcare professional decides whether telehealth is appropriate, whether more information or testing is needed, and whether any treatment or prescription is clinically appropriate. No treatment, prescription, or outcome is guaranteed.
Laboratory and Pharmacy Services
A provider may require laboratory testing. Laboratory services may be performed through third parties including Labcorp, and at-home blood collection may use Tasso devices where offered. Prescriptions may be fulfilled by participating pharmacies, currently including Vios Compounding Pharmacy where applicable. These services are subject to availability, provider direction, and law.
Privacy and Electronic Communications
Telehealth requires electronic handling and transmission of information. Reasonable safeguards may reduce but cannot eliminate all privacy and security risks. Independent providers, patient portals, and other covered entities may provide separate privacy notices, including applicable Notices of Privacy Practices. Review the OptimizeMD Privacy Policy and any provider-specific notices presented during care.
Financial Responsibility
Services are cash-pay, and OptimizeMD does not currently accept insurance for these services. You are responsible for charges disclosed before purchase, including applicable recurring charges, laboratory costs, consultation fees, and medication costs.
Withdrawal of Consent
You may withdraw consent to receive telehealth services as permitted by law by notifying the healthcare provider or contacting support@optimizemd.co. Withdrawal does not undo care already provided and may limit continued access to remote care. A provider may still recommend in-person care.
Acknowledgment
By affirmatively consenting during an enrollment or care workflow, you acknowledge that you understand the nature, potential benefits, limitations, and risks of telehealth; have had the opportunity to ask questions; and consent to receive telehealth services from an independent licensed healthcare professional.
